A typical day here is full but not rushed. Mornings usually start with yoga or breathwork before breakfast, followed by a workshop led by Dr. Molina or one of the facilitators. These sessions cover mindfulness, emotional healing, mind management and nature therapy, and they are practical rather than abstract. You will be asked to look at patterns you have carried for years, and you will leave with tools you can actually use at home. Afternoons mix in sound healing, plant medicine practices, Vipassana-based meditation and free time. The island tour, massage treatments and swimming are built into the schedule so the week does not become purely clinical.

The setting is the main thing that separates this from a city clinic or a standard wellness resort. La Estancia sits on a private island in Busuanga, Palawan, which Travel + Leisure readers voted the World's Best Island in 2013, 2016, 2017 and 2020. Guests regularly kayak through tropical mangroves to a secluded spot for yoga, or take a local bangka boat to a neighbouring island for meditation. There is a shipwreck you can swim over, and white sand beaches within easy reach of the rooms. Being this removed from normal life is deliberate: it removes the distractions that usually make deep work impossible.

Dr. Noreen Molina is a graduate of the University of Santo Tomas and trained at the National Center for Mental Health in the Philippines. She has worked across the full spectrum of psychiatric treatment, including confinement, electroconvulsive therapy, psychotherapy and rapid tranquilization, and now combines that clinical background with the holistic practices used at the retreat. She is supported by Stephen Yanga, a yoga instructor, sound healer and plant medicine guide trained at Yogmantra Bali who specialises in Ashtanga, Pranayama, Hatha and Vinyasa, and by Mac Balbin, a sound healer and Vipassana practitioner who has studied with respected guides and gurus.

The programme is medically supervised, which matters if you are coming off or adjusting medication, or if you have a history of serious mental health issues. Each guest submits an individual application before booking, and there is a pre-retreat call with both the facilitator and the psychiatrist to talk through your needs. Places are capped at eight people per retreat, and applications are processed Monday to Friday, usually within two to three business days.

What most people say afterwards is that four days was enough to shift something. The combination of a psychiatrist who knows what she is doing, facilitators who hold the space well, and an island that forces you offline is hard to replicate elsewhere.
