Piešťany sits about an hour north-east of Bratislava, on the Váh river, and the whole town functions around its spas. The Spa Island — a green park with the hotels, the pools and the treatment houses — is where you'll spend most of your time. There are more than 1,620 mineral springs in Slovakia and 26 thermal and health spas, but Piešťany is the biggest balneotherapy centre in central Europe, and the six hotels on this listing share the same source of thermal mineral water and healing mud.

Treatments are prescribed after an initial consultation with a spa doctor, who reviews what you're dealing with — musculoskeletal issues, cardiovascular conditions, skin problems, or general recovery — and then sets a programme. Typical days include 2-4 treatments, with mornings usually busier and afternoons free for the pools, a walk, or a trip into town. Standard therapies include thermal water bathing, mud packs, paraffin wraps, underwater massage, electrotherapy, and classic Swedish or reflexology massages.

The hotel options differ mostly in style and price rather than in the treatments themselves. Thermia Palace is the grand Art Nouveau five-star on the island, built in 1912, with the best restaurant and the most polished service. Esplanade is a large four-star with a big events area and multiple pools. Splendid is a solid three-star with direct access to the Balnea Health Spa. Pro Patria is the most affordable, with recently renovated rooms and an emphasis on Slovak and international cuisine. Smrdáky is a separate spa about an hour away, best known for dermatology — it's been nicknamed the 'Dead Sea' of Slovakia — and it has its own children's treatment building for ages 3 to 18.

Beyond the spa itself, Piešťany has a golf course and driving range, and the surrounding Váh river valley is flat and good for cycling. There are tennis and volleyball courts in Smrdáky, plus table tennis and billiards. The town has a few cafés and restaurants worth an evening, and Bratislava is day-trippable if you want a change of scene.
